Ready2Nurse is led by Nurse Practitioners. Our nurse practitioners perform all initial patient assessments, developing and overseeing the plan of care. R2N NPs act as liaisons between doctors and case managers, promoting efficient and comprehensive provider communication. We quickly identify any changes in our patients’ condition and determine when other services are needed.
